It cannot be quantified, recorded, accessed, or tagged, but there is a special kind of bond that can be formed between a nurse and her patient. This relationship is formed through care, intimacy, and kindness that turns an average phone call into a treasured moment.
One of the CCM nurses says that every month is her sunshine month because she is lucky enough to call a patient whose voice is cheerful, full of happiness, and who is always smiling. It belongs to a previous patient, an elderly woman who is now 85 and always says, ''Thank you'' the moment the greeting is over.
Then she said, “Thank you for calling.” Before the nurse could even get the chance to ask, she said, “I prayed for you.”
One day, a patient said she got a little scared because the nurse's phone call was very overdue. “I was worried something had happened to you, so I started praying for you a lot.”
There is no chronological order to love like this, and no one to guide you to it. It is very difficult to do, but it is what differentiates real love from the rest.
We began each meeting with a segment on health updates that included prescriptions, symptoms, and follow-ups, but this quickly turned into something more meaningful.
They talk about families, their faith, and the little things from each day. Before the end of the call, the patient always, always says with much feeling:
"I love you and pray for you every day."
You aren't just comforted by their words; your spirit is lifted. Kindness goes both ways. Kindness comes back; Kindness is returned. 'She is 85. She is a precious person. When I'm having a rough day, she makes everything better. She makes my day at work wonderful,' says a nurse.
In today's ever-changing world, healthcare is just another industry where it's being augmented with tech, AI, and data to improve workflow. However, a reminder that we must connect, not with machines, is most important. Enable Healthcare Chronic Care Management programs provide structure and consistency; it is people like her that give it life, give it heart.
What none of us processes on every CP update, every patient call, every care plan is that behind our screens is a genuine person who quietly and patiently says, 'We were praying for you'.
Why Stories Like This Matter
In healthcare, like in many other fields, it is not only medical accuracy that is most important, but emotional accuracy that is most necessary. When a patient is given more than just their medical file and feels that they are truly being superbly represented, their well-being is prioritized, and they experience improvement, medical and otherwise. Compassionate care and sentiment foster trust, increase follow-through, and turn concern into connection.
And that's the essence of Chronic Care Management (CCM). It fosters the combination of clinical care and personal connection, engaging patients, and reminding nurses of their passion for the profession.
