Chronic Care Management (CCM) nurses know that every conversation can uncover early warning signs that lead to life-saving care. Listening closely, asking the right questions, and taking quick action can make all the difference.

It began on October 7, during a routine CCM outreach. The nurse checked in with a patient who casually mentioned experiencing left-sided abdominal pain and nausea. Though the patient didn’t sound alarmed, the nurse immediately recognized that these symptoms could indicate something serious — possibly intestinal obstruction or appendicitis.
With calm but firm guidance, the nurse urged the patient to visit the emergency room right away for evaluation. The patient listened carefully and agreed to seek care that same day — a sign of trust and strong engagement in her own health.
A few days later, the nurse followed up to see how the patient was doing. The patient shared that she had indeed gone to the ER and was diagnosed with a left-sided abdominal mass. She had been discharged with instructions for further testing and was scheduled to see a gastroenterologist for specialist evaluation.
