Remote Patient Monitoring 

The Remote Patient Monitoring program allows your Doctor to monitor readings you take at-home. The program is designed to give you and your Doctor more information on your health and, allows them to make better decisions on your medication regimen.

This is how it works.

The remote patient monitoring program gives you more opportunities to interact with your Doctor. It improves your chronic condition management. You can receive help booking appointments and medication refills with our nurses. Plus, Doctors can monitor your medications efficacy and review your readings monthly.

You can take and transmit a reading all with a touch of a button. With built in GSM, there is no need for Bluetooth hubs, cellphones, or Wi-Fi dependent systems. 

Just take your regular reading and your Doctor will be able to view it. It's that simple.

Your Responsibilities. Remember to take readings at least once a day or otherwise indicated by your Doctor.
