GIFT CARD CONDITIONS*

01. REDEMPTION 

 

Gift Cards will be redeemed via www.thegiftcardshop.com OR www.giftcards.com 

 

02. LIMITATIONS 

 

In order to be eligible for the gift cards you must have a licensed medical practice in the United States with a verified email address, phone number, NPI and physical address. 

 

The Demo must be marked as completed by Enable Healthcare Inc.'s Product Specialist in order to receive the $25 gift card. If a licensed Physician completes the Demo you are eligible to receive a $25 gift card. If an office manager completes the Demo you are eligible to receive a $15 gift card. 

 

Your account must be active for a total of 90 days with Enable Healthcare Inc. in order to receive the $200 gift card. 

 

Gift Cards/Certificates may not be redeemed for the purchase of products from Enable Healthcare Inc. 

 

Gift Cards/Certificates cannot be used to purchase other Gift Cards/Certificates.

 

Gift Cards/Certificates cannot be reloaded, resold, transferred for value, redeemed for cash or applied to any other account, except to the extent required by law. 

 

Unused Gift Card/Certificate may not be transferred.

 

03. OUR POLICIES 

 

Gift Cards/Certificates and their use on the Shop are subject to Enable Healthcare Inc.'s General Terms and Conditions and Privacy Statement. Enable Healthcare Inc. may provide Gift Card/Certificate purchasers with information about the redemption status of Gift Card/Certificates.

 

04. RISK OF LOSS

 

The risk of loss and title for Gift Card/Certificates pass to the purchaser upon our electronic transmission to the recipient or delivery to the carrier, whichever is applicable. We are not responsible for lost or stolen Gift Cards/Certificates.

 

05. FRAUD

 

Enable Healthcare Inc. will have the right to close customer accounts and request alternative forms of payment if a fraudulently obtained Gift Card/Certificate is either redeemed  or is redeemed and used to make purchases.

 

06. LIMITATION OF LIABILITY 

 

Enable Healthcare Inc. and its affiliates make no warranties, express or implied, with respect to Gift Cards/Certificates, including without limitation, any express or implied warranty of merchantability or fitness for a particular purpose. In the event a Gift Card/Certificate code is non-functional, your sole remedy, and our sole liability, shall be the replacement of such Gift Card/Certificate. Certain country laws do not allow limitations on implied warranties or the exclusion or limitation of certain damages. If these laws apply to you, some or all of the above disclaimers, exclusions, or limitations may not apply to you, and you might have additional rights.
